The Registration Form should be filled as soon as possible. Please note that you are not completely registered for the workshop until we have received the payment.
Registration Fee
All prices are in Czech Crowns (CZK), see the exact current exchange rates (fluctuating around: EUR 1,- = cca. CZK 25,-).

The regular and student fees include the abstract book, welcome reception, coffee breaks, 4 lunches, 2 dinners, 1 banquet and excursion. Students should be able to prove their student status. The accompanying person fee includes the welcome reception, 2 dinners, 1 banquet and excursion.

- by a credit card (mail order) - VISA, EuroCard/MasterCard, JCB, Dinners. (Sorry, American Express credit cards are not accepted.) In case of credit card payment, please print and fill in clearly a completely the Payment Form and fax it to +420 272 934 120 .

In case of bank transfer, please make sure it is made net of all bank charges and commissions. Please note that to proceed the transfer correctly you have to include all these identification items and indicate the name of participant(s) in the comment.
A confirmation of receiving your payment will be sent to you by e-mail. All payments should be realized before the conference to guarantee all conference services. Payment at the registration desk is possible in cash (CZK) only.
Cancellation Policy
75% of the registration fee will be refunded for all written cancellations postmarked no later than July 15, 2012. After that date no refund of registration fee will be provided.
